Consent Form Guidelines [pdf]
New federal guidelines require certain key information to appear at the start of the consent form. Carefully review the guidelines before creating your consent form(s) so that you understand all of the essential elements that must be included.

Consent Form Template for Adult Participants [dotx] - Word fillable template
Use this template when preparing a consent form for adult participants. Save the file as a Word document using your last name, i.e., Smith Consent Form for Adult Participants.

Consent Form Template for Participants Under Age of 18 [dotx] - Word fillable template
Use this Consent Form when seeking consent from a parent or guardian to have a minor participate in your research study. Save the file as a Word document using your last name, i.e., Smith Consent Form for Parents.

Research Supervisor Form [dotx] - Word fillable template
All students must complete this form with their research supervisor and submit with their application. Save the file as a Word document using your last name, i.e., Smith Research Supervisor Form.

Project Continuation or Modification Form [dotx]
All research is initially approved for one year. Any change to an approved research protocol requires the submission of this form. A modification approval within the original timeline of the project does not change the approval termination date. In addition, any approved research that goes beyond one year requires the submission of this form. Renewed research applications are approved for one year.
